By Sunday evening of the last weekend in June, the Codex war room had already burned a Friday investigation and a Saturday on-the-house flush. Tibo still did not have a single villain. He reset everyone anyway — a hard reset — and noted, because the universe has a copy desk, that the week at OpenAI was called RESET week.
The Friday status incident was dry: Codex usage limits depleting faster than expected. By Saturday Tibo said mitigations were in and the blast radius did not look company-wide, then reset the house anyway. Sunday's note was colder. Still investigating. Hard reset. Some of you had three banked grants. Those are gone.
Monday night he published the autopsy. There was no central bomb. Auto-review had gotten eager. Subagents were doing extra laps. Background suggestions could run twice or retry like a slot machine. The usage chart was lying in a smaller way — failed and rate-limited turns drawn as if they counted, auto-review wearing a GPT-5.4 badge. The abuse and fraud stack, OpenAI later said, had also been rate-limiting the wrong people hard enough that some paying seats looked 10–20× hungry.
The remedy was three flushes in three days and one extra banked reset credited for the next 24 hours. That is how you can tell an investigation from a party: the house keeps hitting the button because the first press did not fix the pipe.
We keep 28 June as a lead because it is the event that taught the culture the expensive sentence: a hard reset is not a gift if you were saving coupons. Print the quote. Print the date. Do not sand it into a cute heatmap square.
